The Illinois form a seller uses to tell the Secretary of State a vehicle has been sold. Fill it in here and it downloads as a finished PDF of the state's own form, ready to sign.
It is not a bill of sale, and it does not transfer ownership. The form says so itself: ownership moves through the Assignment of Title on the certificate of title, and the buyer then applies for a new title.

The seller's signature and the “Executed on” date are left blank. The form is a statement under penalty of perjury, so those two are the seller's to write. The form carries the Secretary of State's mailing address.
